It really depends on how high the fifth wheel you buy is. Heights vary depending on manufacture, model, and size. I would go with at least 13'.

My dad has a Wildcat that is called mid profile and it will not fit under his 12' door so I would say go for 14' and never worry about it.

Our unit is 12'8". I'm 6'2 and it is barely tall enough that I can stand up straight in the bedroom. I think a 12' door is low for many if not most 5ers

You will need a 14' wall opening to ensure any fifth wheel will fit into it. Consider a sliding door which will eliminate any additional wall height.
